Student requests for Personal Protective Equipment (PPE)
The College of Health Solutions is dedicated to ensuring that students participating in a for-credit course have the proper Personal Protective Equipment (PPE) during the COVID-19 pandemic. The College of Health Solutions will provide PPE free-of-charge to any student enrolled in a CHS degree program who is participating in fieldwork, internship, or research where PPE is required, but not provided by the site. It is the responsibility of faculty members to provide PPE for students participating in research in a faculty-led lab.
To request PPE, follow the process outlined below:
- Complete all fields and digitally sign the Student Request for Personal Protective Equipment (PPE) form.
- Attach an email or other documentation from your site supervisor or preceptor indicating what PPE is required.
- Once the request is processed, you will be notified via email when your PPE is available for pickup.
- Pick up your PPE at Health North on the Downtown Phoenix campus or Coor Hall at the Tempe campus. If you are an online student or do not currently reside in the Phoenix area, alternative arrangements may be made on a case-by-case basis.
